NURS 455 - Continuum of Care (4 S.H.) (3 S.H. theory/1 S.H. practicum) (Baccalaureate RN Completion)
This course explores complex continuity of care issues involving the individual, family, and/or community. The content will assist the student to perceive the significance of professional evidence-based nursing interventions related to specific situations and/or populations. Some of the diverse situations include: older adults, people with dementia, people at the end of life, people with mental illness, and culturally diverse individuals. Evidence-based teaching-learning theory and skills are applied to the adult learner population. Prerequisites/co-requisites: Admission to RN-BS Completion Program, ◆ NURS 375 - Research and Evidence-Based Nursing Practice , NURS 415 - Concepts in Professional Nursing , NURS 435 - Holistic Assessment , NURS 465 - Population-Based Health Care II . Prerequisites or co-requisites: △ NURS 445 - Evidence-Based Leadership and Management , △ NURS 475 - Population-Based Health Care I , ◎ NURS 491 - Professional Development . Grade only.
